I understand that touch-ups may require additional charges unless otherwise stated.

I understand deposits are NON-REFUNDABLE and secure artist time and scheduling.

I acknowledge that no guarantees have been made regarding exact healed results, colors, or longevity.

I consent to the application of the tattoo and any actions or conduct reasonably necessary to perform the procedure safely.

Tattooing persons under the age of 18 years of age is prohibited, except under limited legal exceptions involving the covering of existing offensive or prohibited tattoos with parental consent.

Tattooing individuals under the influence of drugs or alcohol is prohibited.

I voluntarily release and forever discharge The Hive Collective, its owners, artist, employees, and affiliates from any and all claims, damages actions, causes of action, or liability arising from my tattoo procedure, including but not limited to negligence, allergic reactions, infection, fading, dissatisfaction with artistic interpretation, or failure to follow aftercare instructions.
